Quick summary
- Washington’s published NLV checklist requires a physician, excluding nurse practitioners and physician assistants.
- The bilingual template can avoid a separate translation, but must be completed correctly.
- The certificate’s age is assessed against the visa application date, not the departure date.
A routine clinic letter may not be enough
Your usual primary-care provider may be a nurse practitioner, and the clinic may offer a standard letter confirming that you are healthy. For this application, check who will actually sign before booking.
The Washington NLV checklist, section 14, specifically excludes a Nurse Practitioner or Physician Assistant as signatory. This is a published document requirement for Washington’s procedure. Applicants in another Spanish consular district should follow their own office’s instructions rather than assume every US post uses identical wording.
If you already have an NP-signed certificate, contact the clinic about obtaining one issued and signed by a physician. Do not assume that a clinic administrator’s stamp or a Spanish translation will correct the signatory problem. Nor should you assume that simply adding a countersignature will be accepted; the physician must be able to issue the certificate, and any uncertain format should be clarified before submission.
What the physician needs to certify
Article 38(i) of Royal Decree 1155/2024 requires a medical certificate addressing diseases with potentially serious public-health repercussions under the International Health Regulations of 2005. A generic statement that you are fit to travel does not address that specific requirement.
Send the clinic the bilingual medical certificate linked by Washington BLS in advance. It contains the required public-health statement in English and Spanish, spaces identifying the applicant, the place and date, and fields for the medical-centre stamp, physician’s signature and licence number. The physician determines whether they can make the certification; applicants should not complete the medical finding themselves.
Washington’s checklist asks for the original and a copy. Before leaving the clinic, compare the completed document with the template and check your name, date, signature and identifying details.
The bilingual form saves a separate translation
Washington accepts a certificate in Spanish, one including the Spanish text, or one accompanied by an official or sworn Spanish translation. Its checklist says a separate translation is unnecessary when its model is used.
That makes the bilingual template useful, but it only resolves the language requirement. It does not excuse an unsuitable signatory, missing details or an old certificate. If the clinic issues its own English-only letter instead, arrange the required translation and verify that the letter still contains the specific public-health statement.
Time the certificate around submission
Washington requires issue within the 90 days before the visa application. Count from the planned submission date rather than your flight or intended arrival in Spain. If the appointment moves, check the certificate’s age again.
For example, in a hypothetical application, an applicant obtains a physician-signed certificate 80 days before the appointment. A three-week postponement takes it outside the published 90-day window. Correct wording and a valid signature would not solve that timing issue.
ANEMOSS ASESORÍA JURÍDICA S.L. can help organise the personal documents and check their fit with the relevant checklist as part of its Non-Lucrative Visa service. The separate FBI-check guide covers criminal-record evidence; its document and authentication requirements should not be confused with those for this certificate.
Common mistakes
- Booking a routine clinic visit without checking whether a physician will issue and sign the certificate.
- Translating an NP-signed certificate instead of resolving the signature problem.
- Calculating the 90-day window from travel rather than visa submission.

Frequently asked questions
Can a physician assistant sign instead?
No. Washington’s published checklist excludes physician assistants as well as nurse practitioners.
Does my health-insurance certificate replace this document?
No. The Ministry’s initial NLV guidance lists health-insurance evidence and a medical certificate separately. Both serve different purposes.
